Clare Hall art collection form

This form will be filled in as you collect the work you want to borrow from the Porters’ Lodge. You will be contacted by a member of the Art Committee when the work is ready for collection. Before the work can be taken to your room you will need to fill in the details and sign the clause below. Please complete a separate form for each work borrowed. NB: This form will be filed for the duration of your loan. Please also note that when you leave the College, the work must be returned to the Porters’ Lodge and the red book listing all the loans must be signed to acknowledge its return. I hereby accept to return the loaned item on the due date and in the same condition as at the beginning of the loan period and accept full responsibility for any damage caused while the work is in my care.(Required)
